Individual Learning Plan

This is an Individual Learning Plan, which I created for Student B - Jai with limited assistance from my mentor teacher. It focuses on the areas of personal learning, literacy and mathematics, and ideally these goals are to be met by the end of the term.

Personal Learning -

Smart Goals:

- I will maintain independence during learning so that I can stay on task.

Entry Skills :

- Jai is beginning to gain independence to start work straight away.

- He is reliant on adult assistance in order to maintain focus on a task.

- With repetition, over a period of time, Jai is able to complete short activities with developing independence.

Outcome Target:

- Complete an activity within the 10minute time allocation.

- Maintain focus on the task for 10 Minute time allocation.

- Pursue any activity independently for a short amount of time

School Strategies:

- Reinforce expectations of working for the whole time when time is active.

- Reward

Home Support:

- Set tasks for 10 minutes and set expectations that it is work time.

- Use terminology ‘work time’ and ‘choice time’ to support home school learning.

Literacy -

- I will use known high-frequency words so that I can independently demonstrate my knowledge in my writing.

Entry Skills:

- Jai can spell 26 of the oxford 100 words.

- He is beginning to recognise these words when reading own readers.

- Jai is now able to use some oxford words in context when writing, with encouragement.

Outcome Target:

- Jai will begin to use these words in context when writing independently.

School Strategies:

- Support from volunteer learning mentor to practice words several times each week.

- Locating target word sin levelled texts.

- Ordering words to create sentences.

- Attempting to use words when planning and writing sentences.

Home Support:

- Practice flashcards of target words.

- Word games – bingo and snap

- 6 personal spelling words to reinforce and extend his vocab.

Mathematics -

Smart Goals:

- Count and order numbers independently beyond 30.

- Bundling sticks into groups of 10 to assist with skip counting and beginning knowledge of place value.

Entry Skills:

Jai can count to 29.

- Jai can gather a collection of an amount but is unable to separate it into 10’s and 1’s.

- Jai does not use skip counting as an effective method to reach a total quickly.

Outcome Target:

- Automatic recall of numbers 1-40.

- Order number cards of numbers 1-40.

- Skip count by 10’s using materials as a support, initially to 50 and moving towards 100.

School Strategies:

- Explicit practice everyday using target numbers.

- Flashcards.

- Working with volunteer learning mentor on target numbers.

- Materials and modelling – bundling.

- Rhymes/songs to help him to recall the skip counting pattern.

Home Support:

- Use 100’s chart to play “I’m thinking of a number…”

- Counting practice of target numbers.

- Using pencil and paper to play guess this number.

- Skip counting by 10’s.
